The symptoms of gastroesophegeal reflux disease

Objectives:

Gastroesophegeal reflux disease were determined with 24 hour pH monitorization in a selected group of patient and laryngopharyngeal symptoms frequency of them were evaluated with the current literature.

Methods:

The study included 27 patients (15 male, 12 female; range 22 to 82 years; mean age 48.1 years) with the diagnosis of gastroesophageal reflux disease via 24 hour pH monitorization and 10 patients (6 male, 4 female; range 27 to 67 years; mean age 44.5 years) as control group. Both patient groups were monitorized for 24 hour and distal esophageal pH was measured periodically once every 4 seconds. Decreasing of the distal esophageal pH less than four was accepted the beginning of the reflux. Contrary, increasing of the distal esophageal pH more than four was accepted the ending of the reflux. The percentage of the time, in which the esophageal pH is less than 4 during which the patients were standing and in the supine position, to the total time was determined and compared. In addition, laryngopharyngeal symptoms of the gastroesophageal reflux disease were evaluated in the patient group.

Results:

In the control group, The percentages were 7.14% in standing position, 4.83% in supine position and 5.42% for the total time. The percentages for the 27 patients were 27.5%, 30.5% and 28.3%, respectively. In the patient group, 17 patients (62.9%) had heartburn and/or regurgitation, 14 patients (51.9%) had cough, 18 patients (66.6%) had hoarseness, 22 patients (81.5%) had frequently throat clearing, 16 patients (59.3%) had excessive secretion to the throat, 14 patients (51.9%) had dysphagia, 21 patients (77.7%) had globus and 2 patients (7.4%) had dyspnea.

Conclusion:

We must remember probable gastroesophageal reflux disease with the existence of atypic symptoms such as hoarseness, cough, dysphagia, globus, excessive secretion to the throat and frequently throat clearing probable and we must perform 24 hour pH monitorization for the diagnosis.

ÖZET

Amaç:

24 saatlik pH monitörizasyonu ile gastroözofageal reflü hastalığı teşhis edilen seçilmiş hasta grubunda mevcut larengo-farengeal semptomların sıklığı literatür eşliğinde değerlendirilmiştir.

Yöntem:

24 saatlik pH monitörizasyonu ile gastroözofageal reflü tanısı konan 27 hasta (15 erkek, 12 kadın; yaş aralığı 22-82; ortalama yaş 48.1) ve 10 hastadan (6 erkek, 4 kadın; yaş aralığı 27-67; ortalama yaş 44.5) oluşan kontrol grubu dahil edildi. Her iki hasta grubu 24 saat boyunca monitörize edilerek her dört saniyede bir distal özofageal pH'sı değerlendirildi. Özofageal pH'ın dördün altına düşüşü reflünün başlangıcı, dördün üstüne çıkması reflünün bitişi olarak kabul edildi. Hastaların yatar pozisyonda ve ayakta özofageal pH'larının 4'ün altında olduğu zamanın total zamana yüzdeleri tespit edildi ve karşılaştırıldı. Ayrıca 27 kişilik gastroözofageal reflü tanısı alan hasta grubundaki mevcut larengofarengeal semptomlar değerlendirildi.

Bulgular:

Kontrol grubumuzdaki sağlıklı kişilerin, pH'ın 4'ün altında olduğu pH zaman yüzdeleri ayakta 7.14, yatar pozisyonda 4.83, total için 5.42 şeklinde tespit edildi. 27 kişilik hasta grubunda bu değerler sırasıyla 27.5, 30.5 ve 28.3 şeklindeydi. Yirmiyedi kişilik hasta grubumuzda göğüste yanma ve/veya regürjitasyon 17 hastada (%62.9), kuru öksürük 14 hastada (%51.9), ses kısıklığı 18 hastada (%66.6), sık boğaz temizleme 22 hastada (%81.5), boğaza aşırı sekresyon 16 hastada (%59.3), kronik boğaz ağrısı 16 hastada (%59.3), disfaji 14 hastada (%51.9), globus 21 hastada (%77.7), dispne 2 hastada (%7.4) saptandı.

Sonuç:

Ses kısıklığı, kuru öksürük, disfaji, globus hissi, kronik boğaz ağrısı, boğaza aşırı sekresyon ve sık boğaz temizleme gibi atipik semptomların varlığında, gastroözofageal reflüden şüphelenip tanı için 24 saatlik pH monitörizasyonu mutlaka yapılmalıdır.
