Between July 1996 and May 2000 123 patients were operated for rhinoplasty in otorhinolaryngology clinic. 68.3% (n=84) were operated by the closed technique and 31.7% (n=39) were operated by the open technique. Open technique was preferred by the complicated refinement of the nasal tip, secondary revision surgery, when trauma has disrupted the nasal tissue and made it difficult to differentiate, where use of grafts are needed and of the education of trainee surgeons. Endonasal approach is preferred in simple and limited cases and by the experienced surgeons. Techniques for the approach to the nose is only the beginning of the surgery. The surgeon has to choose the technique, that he can manage and prefer. This technique must provide the best exposure, the best way of management and the least complication. Therefore refusal of a technique and supporting another is not an objective behavior for a surgeon, who is a scientist as well. External approach provides new opportunities and safety for the inexperienced surgeon. If there is indication and if the way of management with the complications is known, it can be chosen. Open technique is also important in a otolaryngology clinic, because it helps the education of the candidate surgeons.

ÖZET

Temmuz 1996 ve Mayıs 2000 tarihleri arasında kulak burun boğaz kliniğinde rinoplasti operasyonu yapılan 123 hastanın %68.3'üne (n=84) kapalı, %31.7'sine (n=39) ise açık teknik uygulanmıştı. Açık teknik, komplike burun tipine müdahale gerektiren durumlarda, sekonder revizyon cerrahilerinde, travmanın burun yapılarını bozduğu ve dokunun ayırt edilmesinin zorlaştığı durumlarda, greft kullanımına ihtiyaç duyulduğu hallerde ve genç cerrahların eğitiminde seçilmiştir. Endonazal yaklaşım ise ağırlıklı olarak basit ve sınırlı olgularda ve tecrübeli cerrahların opere ettiği durumlarda tercih edilmiştir. Rinoplasti operasyonunda yaklaşım teknikleri sadece ameliyatın başlangıcını oluşturur. Önemli olan cerrahın rahat olduğu ve uygulamasını bildiği tekniği seçmesidir. Bu teknik hem de hastanın sorunu çözmede en uygun görüşü hazırlayıp, en doğru uygulamayı sağlayacak ve en az komplikasyonu yaratacak teknik olmalıdır. Dolayısıyla herhangi bir yöntemi dışlayıp başka bir girişimin taraftarlığını yapmak hem de bir bilim adamı olması gereken cerrah için nesnel bir düşünce şekli değildir. Eksternal yaklaşım tekniği de rinoplasti cerrahisinde operatöre yeni olanaklar sağlayan ve tecrübesiz cerrah için daha güvenli olduğu düşünülen bir yöntemdir. Endikasyonları doğru konduğu ve komplikasyonları ile mücadele etmesi bilindiği sürece uygulanmasında sakınca yoktur. Bir eğitim hastanesinde ise açık tekniğin ayrı bir önemi vardır. Eksternal yaklaşım sayesinde yeni kulak burun boğaz uzmanı adaylarına rinoplasti eğitimi çok daha kolay verilebilir.
